	Alverno Alumnae Award Nomination Form

The Alverno Alumnae Association Awards Committee is seeking nominations for its 2008 awards. There are five award categories—Sister Joel Read Outstanding Alumna of the Year, Spirit of Alverno Award, Professional Achievement Award, Young Professional Achievement Award, and Service to the Community Award. These awards will be presented at Homecoming on Saturday, October 4th, 2008. The specific criteria for each of these awards can be found at: http://www.alverno.edu/alumnae/awards.html.

Please complete your responses with as much specific information as you have available including concrete examples to demonstrate the nominee's contributions and accomplishments. The committee's decision is based solely on the information provided by you. In addition to your responses below, please feel free to send other materials-letters of recommendations, articles about the nominee, a resume or vita—in support of your nominee.
